Top: in Paper Mario: The Thousand-Year Door, it is possible to lose HP in the overworld by repeatedly falling into water (the screenshot shows Mario immediately after the game starts, before he can get into a single battle, with 1 HP from dropping into water). This can even cause Mario to get a Game Over if he does so while at 1 HP. There is one place in the game where this was seemingly taken into account and a special script inserted to not let it cause Mario to lose his first battle.Bottom: when the first battle against Lord Crump starts, Mario’s HP is manually set to 5 if it is lower than 5 in order to not let him lose (top left corner). Crump has 5 HP and 1 Attack, and goes second after Mario; whereas Mario deals either 1 or 2 damage every turn or can defend, which reduces the damage dealt by Crump for that turn to 0. Note also that Mario cannot appeal or use items, as these options are not available at that point. As such, there is no way for Mario to lose all 5 HP during the battle, as even playing as badly as possible can result only in him losing 4 HP at most due to the variables involved.
